The title is there, it just uses a font colour which is almost identical to the background so is hard to see.
Don't seem to be able to see your attached file though.

I don't see any indication of an attachment. I am writing this reply and selecting 'file' again. Selecting file presented UI for choosing a file, but there was no indication of an upload taking place and yet again no indication that an attachment has been inserted.
